I also want exactly this for my Film Impact transition suite... was trying to figure out a way to do it yesterday and came up dry. Unfortunately it's currently a game of copy/paste from another transition that has been programmed already, which is then a bunch of unnecessary timeline scrolling to go find it.
My current thinking is that I will make a sequence of just my preset transitions and store it in a window really small near the effects panel. So then it's just a visual map of my favorite ones, color-coded. Then, I could program a Keyboard Maestro palette to move the mouse to the appropriate transition, copy it, and paste it where I wanted it. It'd come up in a Palette style, like this one does (I use this script to dynamically adjust the length of the default audio crossfade transition on the fly, so I can quickly change from adjusting dialogue to transitioning music. It cycles all the way through the preferences to adjust it, but it happens pretty instantaneously). Thinking is I'd do the same with a modifier to the apply transition shortcut, but instead of routing through and setting the default transition, it would just copy and paste it from a "library" that I would define. Would rely on found screen images unfortunately, thus it could be improved with Excalibur support, but seems like the Premiere API is the issue so... 🤷♂️ overcoming Premiere with Keyboard Maestro scripts is basically half my day job. 😂
